This all works via hooks for Claude Code, OpenCode, and PI.
When the coding agent revises a plan after your feedback (i.e. annotations), you no longer have to reread the whole thing to guess out what changed in the plan. Plannotator's "Plan Diff" shows exactly what's different about the plan.
